That particular dialog is apparently an unfinished solution for reporting issues to the user. Clearly it was an early-stage quickie that perhaps in the rush to ship they forgot to go back to put in the UI they really meant to.
Do you (or anyone else here) have a simple repeatable recipe for getting to that dialog? I'm sure the team would want to dress that up once it's brought back to their attention.
