How can I add simple graphics

In the days of RevMedia, there was a demo stack that allowed simple line drawings to be created on a blackboard (window).
I am after a similar effect where a photo is placed in the image rectangle and the user of the aplication can draw simple shapes over the photo as annotation.
Example, a circle around someones head with a caption saying "this is Mike!!!"

can anyone come up with the old stack or similar?
